Provider Referral Intake

To help us schedule your patient's appointment as quickly as possible, please upload the following:

• Patient demographics (including contact information and insurance)
• Provider referral/order
• Relevant medical records (if available)

Please note: A provider referral is required for patients with HMO insurance plans and may also be required by other insurance plans before an appointment can be scheduled.

Once all required documents have been received, our scheduling team will contact the patient promptly to arrange their appointment.
